On September 28, patient Nguyen Van Hieu (53 years old, residing in Mang Yang district, Gia Lai province) was being monitored at Hung Vuong Gia Lai Hospital after emergency surgery for a ruptured appendix.
Ultrasound does not detect appendicitis
Mr. Nguyen Van Hung, the patient's son, said that at noon on September 23, he took his father to the emergency room because of a stomachache.
The doctor ordered an ultrasound, X-ray, and some other tests and diagnosed an intestinal infection. Mr. Hieu was given IV fluids, painkillers, and antibiotics, but his stomach still ached.
At noon the next day, seeing his father in severe abdominal pain, Mr. Hung told the doctor to order a CT scan and discovered that Mr. Hieu's appendix had ruptured, requiring emergency surgery.
The family was quite upset, thinking that the hospital did not detect it in time, leading to a ruptured appendix, endangering their lives.
Therefore, a request has been sent to the Department of Health requesting verification and clarification.
Speaking with Tuoi Tre Online , a representative of Hung Vuong Gia Lai Hospital said that patient Nguyen Van Hieu has a special anatomical structure, unlike normal cases.
Accordingly, the patient's appendix was not located in the right iliac fossa but was located backwards behind the cecum under the liver. Therefore, initial diagnostic and ultrasound measures upon admission did not detect appendicitis.
During the monitoring process, when the patient showed signs of increasing abdominal pain, the doctors performed an abdominal CT scan and discovered a ruptured appendix. The hospital mobilized skilled doctors to perform open surgery to treat the ruptured appendix, clean the abdominal cavity, and place a drain.
Currently, the patient's condition is stable, recovering well after surgery, responding well and can be discharged in the next few days.
Rare disease, difficult to diagnose accurately?
Doctor Bui Truong Giang - deputy director of the hospital - said that medical literature has recorded a small number of patients whose body structure is not in the normal anatomical position.
Therefore, the pain symptoms are not typical, leading to inaccurate clinical and paraclinical examination results. In many cases, when inflammation is detected, the appendix has already ruptured.
In this case, Dr. Giang said that luckily, the patient's appendix was detected and treated early, and the surgery was successful. The patient responded well to the post-operative process and is now gradually stabilizing.
The hospital acknowledged that the initial diagnosis process had difficulty determining the condition of the disease because it was a rare case with anatomical abnormalities. After the incident, the hospital held a professional council meeting to learn from the experience.
Mr. Giang also said that the hospital decided to waive all hospital fees to share with patients and their families.
Talking to Tuoi Tre Online , patient Nguyen Van Hieu said that the pain in the appendix area has now gone away but the surgical wound is still quite painful.
