Emergency surgery to save the life of a newborn with complex congenital malformations

Baby girl D.TM was born at the 39th week of pregnancy, weighing 3.5kg in Vinh Phuc on the morning of February 20. With a diagnosis of pulmonary artery stenosis since the 31st week of pregnancy, immediately after birth, baby M was transferred to Bach Mai Hospital for the first intervention with pulmonary valve dilation technique.

Three days later, the baby underwent a second intervention with a stent placement technique. This is a difficult and delicate technique, especially for newborns, because the blood vessels of newborns are extremely fragile.

Doctors at Bach Mai Hospital successfully intervened in a newborn with congenital heart defects.

MSc. Dr. Nguyen Trung Kien, Deputy Director of the Pediatric Center, Bach Mai Hospital, said that the baby had severe pulmonary valve stenosis and right ventricular hypoplasia, so early intervention was needed immediately after birth, otherwise his life would be in danger.

Regarding the difficulties in this surgery, Dr. Kien said that the team had to intervene twice with the baby because the baby's right ventricle was hypoplastic. Therefore, after dilating the aortic valve, the baby needed to recover. And 3 days later, the stent was placed to ensure the highest safety.

Newborns have low birth weight and weak resistance, and the intervention lasts 1-2 hours with endotracheal anesthesia. In particular, the patient is always cyanotic due to lack of oxygen, so quick and decisive manipulation is needed to open the blood path to the lungs as quickly as possible.

After 2 interventions, on the morning of February 29, D.TM recovered, was healthy and was discharged from the hospital to the joy of his family, parents and the medical team treating him.

According to the recommendation of Dr. Nguyen Thanh Nam, Director of the Pediatric Center, Bach Mai Hospital, through the case of baby D.TM and many other cases that have been successfully treated at the Pediatric Center, it shows the important role of fetal echocardiography. Therefore, in addition to regular ultrasound examination of the fetus, mothers should also have fetal echocardiography to screen for congenital heart disease in the fetus right in the womb to be diagnosed and have timely treatment as soon as the child is born.
