On September 12, the Department of Preventive Medicine (Ministry of Health) requested the Director of the Department of Health of Vinh Phuc province to urgently direct the assessment and conclusion of the cause of the serious accident after hepatitis B vaccination at Vinh Phuc Obstetrics and Pediatrics Hospital, which resulted in the death of 1 child and the critical condition of 1 child.
Accordingly, it is recommended that the Vinh Phuc Provincial Department of Health organize an investigation and hold a meeting of the Professional Advisory Council to assess the cause of serious complications during the use of vaccines to conclude the death case after hepatitis B vaccination, and deploy activities to overcome the incident.
At the same time, the Vinh Phuc Provincial Department of Health needs to review the entire vaccination process and handle post-vaccination reactions to rectify this activity in the area; organize retraining for staff participating in vaccination work if necessary to ensure safety according to regulations.
The Department of Preventive Medicine requested the Vinh Phuc Department of Health to send a report to the Department before September 15.

Previously, on the morning of September 10, two 1-day-old twin brothers, the children of mother D.TY (32 years old, Hoang Dan commune, Tam Duong district, Vinh Phuc province) were vaccinated against hepatitis B by medical staff of the Obstetrics Department, Vinh Phuc Maternity and Pediatrics Hospital.
At around 11:45 a.m. the same day, one child suddenly turned purple, had difficulty breathing, suffered respiratory failure and died. At 3:00 p.m. the same day, the other boy also showed similar symptoms, was given emergency care and transferred to the National Children's Hospital. After emergency care, this boy was out of danger.
