Giant tumor occupies more than 80% of girl's liver

Ho Chi Minh City A 24-year-old girl has a tumor larger than 80% of her liver volume, taking up almost the entire upper half of her abdominal cavity, weighing 3 kg, the largest tumor ever operated on by doctors at Military Hospital 175 .

On January 10, Colonel, Dr. Trinh Van Thao, Head of the Department of Abdominal Surgery, Military Hospital 175, said that the large tumor compressed the blood vessels, was difficult to access in the usual way and was very fragile. Before surgery, the doctors reconstructed the image, accurately calculated the volume, and fully described the condition of the related blood vessels on the 512-slice CT-Scanner system.

The surgical team consulted with Associate Professor, Dr. Le Van Thanh, Director of Digestive Surgery, 108 Central Military Hospital, and decided to resect the right lobe of the liver through the anterior approach. This is a complex form of liver resection, very difficult to perform with many risks of complications during and after surgery, especially liver dysfunction.

After more than two hours of intense surgery, the giant liver tumor was completely removed. The patient was cared for in an early recovery program after surgery, and on the first day he was able to stand, walk, and eat by mouth. After a week, the patient was completely stable and was discharged from the hospital.

According to Dr. Thao, the tumor was identified as a hepatocellular adenoma - a rare benign liver lesion. This is also one of the largest cases of hepatocellular adenoma ever recorded. Usually, this tumor exists alone and is found in young women when using drugs containing estrogen.

Hepatocellular adenomas can cause symptoms of pain, heaviness in the upper right abdomen, reduce quality of life, risk of rupture causing bleeding up to 68% or transformation into malignant liver cancer at a rate of 5%. Currently, this type of tumor is recommended to be operated early at any size, especially when the tumor is 5 cm or larger or has caused complications.

The hospital has successfully performed surgery on several large liver tumors, most recently removing a 1.65 kg right liver tumor from a 78-year-old male patient.
